Simplified DocBook: The Definitive Guide (pre-1.0 but still useful)

The current version of Simplified DocBook is 1.1, and Jeremy recently
made updates to our XSLT to bring it up to this version. I'm not sure
what all the differences are between 1.0 and 1.1.
